We will contact you to schedule your first appointment upon receiving this inquiry.
Step 2
Complete & submit intake paperwork.

All forms must be submitted at least three days prior to your appointment.

All documents are secured through DocuSign.
Step 3
You will be seen by an intake specialist for your initial appointment with our team.
​
There will be a $65 fee, depending on insurance coverage.
