Women’s Health Innovations of Arizona (“WHI Arizona”) is committed to protecting your health information. We are required by law to:

  • Maintain the privacy of your protected health information (PHI).

  • Provide you with this Notice of our legal duties and privacy practices.

  • Notify you if there is a breach of your unsecured health information.

How We May Use and Share Your Information

We may use and disclose your health information without your written permission in the following ways:

  • Treatment – To provide, coordinate, or manage your care (e.g., sharing information with your therapist, physician, or other healthcare providers involved in your treatment).

  • Payment – To obtain payment for services from insurance companies or other payers.

  • Healthcare Operations – For quality assessment, staff training, licensing, and other operations to improve our services.

  • Legal Requirements – When required by federal, state, or local law (e.g., reporting abuse, responding to court orders).

  • Public Health & Safety – To prevent or reduce a serious threat to health or safety.

  • Business Associates – With trusted vendors who help us deliver services (such as billing or IT), provided they also protect your privacy.

Other Uses and Disclosures Require Your Authorization

We will not use or share your information for marketing, fundraising, or other purposes without your written authorization. You may revoke your authorization at any time in writing.

Your Rights Regarding Your Health Information

As a client of WHI Arizona, you have the right to:

  • Access Your Records – Request a copy of your health information. Fees may apply for copies.

  • Request Corrections – Ask us to amend health information you believe is incorrect or incomplete.

  • Limit Disclosures – Request restrictions on how we use or share your information (though we may not always be able to comply).

  • Confidential Communications – Ask us to contact you in a specific way (e.g., only by phone, or at work instead of home).

  • Accounting of Disclosures – Request a list of certain disclosures we have made of your health information.

  • Paper Copy of This Notice – Request a paper copy at any time, even if you received it electronically.

Our Responsibilities

  • We are required by law to maintain the privacy and security of your health information.

  • We must follow the duties and privacy practices described in this Notice.

  • We will not use or share your information other than as described here, unless you give us written permission.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health & Human Services (HHS), Office for Civil Rights. You will not be retaliated against for filing a complaint.
